  • Harassment, discrimination, or bullying.

  • Conflicts of interest or unethical behavior.

  • Financial misconduct or fraud.

  • Aviation safety concerns, including component failures or near-misses.

Likelihood (Probability of Occurrence): Select the likelihood that this event will happen again:
1 - Very Improbable (Practically impossible)
2 - Improbable (Very rare, almost never happens)
3 - Remote (Unlikely but still possible)
4 - Occasional (Has happened before and may happen again)
5 - Frequent (Occurs regularly, has happened multiple times)
Severity (Impact Level): Select the severity of the event:
A - Negligible (Minimal or no impact, minor inconvenience)
B - Minor (Minor injury or financial impact, easily recoverable)
C - Major (Significant business disruption, serious injury)
